From E! - Andy passed away from a five-year struggle with heart disease.

The last time I saw him was 2005 at a SlayerCon here in Knoxville. Looking back on it, it seems like he was sitting a lot during the sessions. Sad, sad day...

From Hollywood.com (from a few years ago; speculation is that the heart failure resulted from the infection):

The end of Angel coincided with another disturbing situation for Andy. Around the time of filming "A Hole in the World," he says, "I remember a filling coming out of a cavity in my mouth. It didn't hurt and I didn't get it fixed immediately, and eventually, it got infected. But it wasn't inflamed, so I never knew that I had a problem - until the day I was in the hospital."

About a month after Angel wrapped, Angel make-up artist Dayne Johnson dropped by to visit, realised his friend wasn't well and wound up taking Andy to the doctor, who sent Andy to the emergency room. The tooth infection, Andy explains, "[had] a direct line to the heart, which was causing the heart to not pump as efficiently, so my heart was getting more and more enlarged. When I finally went in, I was in the middle of congestive heart failure and it was leaking blood from all the valves. The whole thing would be described as advanced cardiomyopathy. But I'm better now - I'm so thankful."

I staggered to my chair with my wake-up cup of coffee this morning and clicked on the tv to see the news. The screen filled up with Lorne's green face singing Somewhere Over the Rainbow. Made me tear up but it also made me smile. So sad that he died so young but I think we can all say that the guy left behind smiles for all of us.
Some days its not even worth chewing through the restraints.

He was a good guy. Great convention goer. He seemed to generally like and appreciate his fans. I knew he had health issues I just didn't realize how bad. He will be missed.

Of the mead so sweet upon our lips,
In those summer days,
All that remains is a jar gone dry,
And a winter memory that stays.

Andy was a favorite of mine on ANGEL and I will miss him. He was a part of some of the funniest stuff ever to appear in the Jossverse.

This is from TV.com:
Actor Andy Hallett passes away at the age of 33 after lengthy battle with heart disease.

It's a melancholy time in the Whedonverse, as one of its own has passed away before his time. Actor Andy Hallett, who played the demon Lorne in Angel, died Sunday night from heart failure at the age of 33, reports E! Online. He had been battling heart disease for five years.

Hallett appeared in almost 80 episodes of the paranormal dramedy, and donned green skin and horns for his role. As Lorne, also known as "The Host" or by his full name Krevlornswath of the Deathwok Clan, Hallett added plenty of comic relief to Angel. Lorne's kind were supposed to be at war with humans, but he instead embraced their arts and served as the emcee at a karaoke bar.

After Angel, Hallett dedicated his time to his music career, playing shows around the country. A private funeral is scheduled later this week in his hometown of Cape Cod.

Andy made it easy being green and now he will "not fade away".
